While in the grand scheme of international nuclear arms control, this is a very small point. However, I was curious as to how this “mistake” happened? How do you miscount? Or, was this just a "paperwork error”? Any insight?

No, I don't know the details. But it's not that unusual - things like that happened in START a few times. As I understand, determining operational status of a bomber could be quite tricky sometimes.
